1. How to Participate
The sponsor of this prize draw is BC Cancer Agency Prevention Programs.
No Purchase Necessary.
1.1 Eligibility
The BC Cancer Agency Prevention Programs – Hi5living.org website FALL 2010 participant Self- Assessment Prize Draw is open to all residents of British Columbia over the age of fifteen (15), except employees, contracted consultants, agents, associates, representatives, advertising and promotional agencies.
1.2 Contest Period
Starting on 12:00 a.m., Pacific Standard Time, Sept. 29 , 2010 to 11:59 p.m., Pacific Standard Time, Nov. 10 2010, participants may enter in the prize draw by completing the revised self- assessment from the Hi5living.org website. This website page: hi5living.org/self-assessment.php - is the only way to enter into the prize draw.
1.3 Contest Entry
Upon completion of the revised self-assessment, eligible participants will be entered into the prize draw for chances to win (i.e. one entry) the prize.
1.4 Contest Entry Restrictions
Participants are restricted to one (1) entry via survey completions per person for the whole period of the contest period (See section 1.2). Entries indicating the same email addresses will be considered, for the purposes of the prize draw, to belong to the same participant and therefore are subject to the above restriction. Any excess entries will be invalid for purposes of the prize draw.
1.5 Awarding of Prize
The Prize will be awarded to the eligible participant whose contact email address has been given by the entrant at time of completion of the survey and who has complied with these Rules.
2. Prize
2.1 Prize
: The prize draw is divided in five (5) prize period as indicated below. There will be five (5) prizes awarded for each prize draw period during the prize draw as follows (twenty-five (25) prizes in total will be awarded during the prize draw):
- Prize Draw Period 1 – on 12:00 a.m., Pacific Standard Time, Oct. 6, 2010 through 11:59 p.m. Pacific Standard Time, Oct. 12, 2010
- Prize Draw Period 2 – on 12:00 a.m., Pacific Standard Time, Oct. 13, 2010 through 11:59 p.m. Pacific Standard Time, Oct. 19, 2010
- Prize Draw Period 3 – on 12:00 a.m., Pacific Standard Time, Oct. 20, 2010 through 11:59 p.m., Pacific Standard Time, Oct. 26, 2010
- Prize Draw Period 4 – on 12:00 a.m., Pacific Standard Time, Oct. 27, 2010 through 11:59 p.m. Pacific Standard Time, Nov. 2, 2010
- Prize Draw Period 5 – on 12:00 a.m., Pacific Standard Time, Nov. 3, 2010 through 11:59 p.m. Pacific Standard Time, Nov. 9, 2010
Each Prize consists of one (1) Chapters, itunes, or Subway Pre-Paid card of CDN $30 ("Prize") only. Participants will be asked in the survey which they prefer, but if no selection is made at that time, the winner will be given another opportunity to choose when contacted via email. There will be a random draw for each Prize Draw (See section 2.3). Each Prize must be accepted as awarded. No Cash alternatives, substitution or transfers of the Prize will be allowed. Each Prize must be forwarded to the winner within three (3) to five (5) weeks from the date when the winner’s signed Declaration and Release Form is received by BC Cancer Agency Prevention Programs.
2.2 Odds
The odds of being randomly selected to win a Prize depend on the total number of eligible entries received during each applicable Prize Draw.
2.3 Prize Draw
The Prizes (eight in total) will be awarded by random computer generated draws (one draw per Prize Draw period, eight in total) from all eligible Prize Draw entries received during the applicable Prize Draw Period. Each draw will be held at 10:00a.m., at Suite 600, 750 W. Broadway, Vancouver, BC, V5Z 1H5 on the following dates ("Draws"):
- Draw for Prize Draw Period 1 – Oct. 6, 2010
- Draw for Prize Draw Period 2 – Oct. 13, 2010
- Draw for Prize Draw Period 3 – Oct. 20, 2010
- Draw for Prize Draw Period 4 – Oct. 27, 2010
- Draw for Prize Draw Period 5 – Nov. 3, 2010
2.4 Notification of Selection
The selected entry will be contacted via email at least three times within the fourteen (14) days after the Draw. If the selected entry cannot be contacted or does not reply to the emails in that time, another entry will be randomly selected and the initial selected entry will be disqualified and will have no recourse towards BC Cancer Agency Prevention Programs or anyone involved in the Prize Draw. In order to receive the prize, the selected entrant will be required to sign a standard Declaration and Release Form confirming compliance with the Prize Draw Rules, acceptance of the Prize as awarded and releasing BC Cancer Agency Prevention Programs from all liability relating to the Prize Draw. No substitution for, or transfer of the Prize, will be allowed.
3. General Rules
3.1 Each Prize claim is subject to verification by BC Cancer Agency Prevention Programs and/or its designated representatives. Entries made with multiple email addresses, under multiple identities, or through the use of any device or artifice to enter multiple times or entries that are in excess of the permitted number of entries during the Prize Draw period are automatically void. All Prize Draws obtained through unauthorized sources or which are incomplete, illegible, mutilated, altered, reproduced, forged, irregular or fraudulent in any way or otherwise not in compliance with these Rules are automatically void. BC Cancer Agency Prevention Programs will be the sole and final reference source in validating a Prize claim.
3.2 By participating, each entrant and selected winner agrees: (a) to be bound by these Rules, and the decisions of BC Cancer Agency Prevention Programs, (b) to release and hold harmless BC Cancer Agency Prevention Programs, its affiliates, subsidiaries, and independent contractors, and their respective directors, officers, employees, agents and representatives including advertising and promotion agencies, from any and all liability for claims/damages including, but not limited to, claims/damages for personal injury, for property damage with respect to acceptance, possession or use of misuse of the Prize or participation in the Prize Draw; (c) to permit BC Cancer Agency Prevention Programs to use his/her name, address, city, of residence, photograph, video, or any likeness for advertising or publicity purposes without any additional compensation, and (d) the winner or winner’s parents agree to sign a Declaration and Release Form to this effect.
3.3 The decisions of BC Cancer Agency Prevention Programs in relation to this Prize Draw will be final and binding on all entrants.
3.4 BC Cancer Agency Prevention Programs shall not be responsible for incorrect or inaccurate transcription or registration of Prize Draw entry information, technical malfunctions, lost/delayed data transmission, omission, interruption, deletion, defect, faulty, incomplete, incomprehensible, or erased computer or network transmissions, line failures of any telephone network, failure of computer equipment, software, inability to access any online service or website, inability to submit the online survey or quizzes, or any other error or malfunction, or any injury or damage to entrant’s or any other person’s computer related to or resulting from participation or downloading any materials in this Prize Draw, or for late, lost, stolen, postage dues, illegible or misdirected entries.
3.5 BC Cancer Agency Prevention Programs reserves the right to cancel, terminate, or suspend this Prize Draw, at its sole discretion, in the event of a technical failure, computer virus or bug, unauthorized human intervention, fraud or any other occurrence or cause beyond its control that corrupts or adversely affects the administration, security, fairness or normal operation of the Prize Draw, subject to BC laws.
3.6 If a selected entrant is awarded the Prize due to a system error, malfunction or defect the Prize will be returned to the Prize pool, to be re-awarded.
3.7 Under no circumstances will BC Cancer Agency Prevention Programs, its affiliates, subsidiaries, and independent contractors, and their respective directors, officers, employees, and agents, including advertising and promotion agencies be required to award more Prizes than what is indicated in these Rules or award the Prizes other than in accordance with the Rules.
3.8 The Prize Draw is subject to all Federal, BC, and local laws, as well as the rules adopted in view of said laws. The personal information provided to enter the Prize Draw will be used only to administer the Prize Draw, and for no other reason. The collection of survey results and email addresses is subject to the privacy guidelines of BC Cancer Agency.
